Friends of the Family Position

MSW Case Manager
This is a unique opportunity to develop and implement a ground-breaking component of Friends of the Family’s participation in the First Five LA Best Babies Collaborative that involves case management and support for women at risk of having poor birth outcomes.

Hours/Salary: Full time, $54,000/year; position may require evening and/or weekends

Duties/Responsibilities including but not limited to:

· Provides case management, service coordination and resource linkage for women at risk of having poor birth outcomes
· Ensures the provision of comprehensive services to participants based on professional assessment of psychosocial status
· Makes home visits to address issues related to promoting healthy pregnancy, post-partum and inter-conception care and assist with daily life management for at-risk women
· Attend all Best Babies Collaborative meetings and workgroups and provide feedback and updates to Friends of the Family staff
· Maintain timely and accurate case files and provide necessary data to Best Babies Collaborative for data entry

Education/Qualifications/Skills:

· Master Degree in Social Work and three years case management experience in related field
· Fluent in oral and written English & Spanish, preferred
· Excellent oral and written communication skills required (please submit a writing sample with your resume)
· Experience in home visits preferred
· Expert knowledge of quality prenatal, postpartum and interception care behaviors, child and family development principles, helpful
· Demonstrated knowledge of community resources & social services
· Knowledge of dynamics of at risk populations
· Problem solving and crisis intervention skills
· Ability to work independently and as part of a team
· Proficiency in Word, Excel, Power Point, Access, Publisher & Outlook
· Must have a car, be able to drive, have a valid CA driver’s license, and current auto insurance
· Satisfactory completion of all background screening requirements

Coalition for Economic Survival Position

Tenant/Community Outreach Organizer
Coalition for Economic Survival (CES)

The Coalition for Economic Survival is seeking qualified people to organize tenants in at-risk affordable housing. The Los Angeles area has one of the most severe housing crisis in the nation. Landlords, in efforts to obtain more profit and higher rents at minimum cost, are constantly attempting to illegally evict tenants, refusing to make needed repairs, converting and destroying affordable housing, and opting out of federal rent subsidy programs. The tenant organizer will identify organizing situations and tenant leaders, develop tenant associations, campaigns and other efforts to preserve affordable housing and secure tenants' rights.

The Coalition for Economic Survival, Los Angeles’ leading tenants’ rights organization, has been organizing people for economic and social justice for the last 35 years.

JOB RESPONSIBILITIES INCLUDE:
• Identifying, developing and supporting tenant leaders, including:
a) training leaders in skills such as meeting facilitation, speech writing and negotiating with owners, management companies and government officials.
b) helping leaders learn how to choose winnable goals, and how to strategize
and implement campaigns to win concrete victories.
c) providing day-to-day strategic advice and basic administrative support including typing, research and writing.
• Organizing and supporting tenant associations, including:
a) door-to-door canvassing, leafleting and speaking at outreach meetings.
b) organizing initial building meetings and ongoing tenant association meetings.
c) preparing meeting notices, agendas, educational materials.

QUALIFICATIONS
Required
1) MUST have the ability to communicate in Spanish.
2) Good writing, speaking and research skills.
3) A valid driver’s license, insurance and a dependable car is a must.
4) Ability to work evenings and weekends. On some occasions work will require irregular and long hours.

Desirable
1) Experience in building tenant, community or labor organizations.
2) Experience in community activism.
3) Working knowledge of word processing and basic computer graphics.
4) Knowledge of landlord/tenant law and housing issues and financing.

SALARY AND BENEFITS
Depends on experience, plus medical and dental insurance.

AIDS Healthcare Foundation Position

We are searching for a Medical Social Worker to work in our
Healthcare Center located in Hollywood California. The hours are Monday-Friday from 8:30 a.m. to 5:30 p.m.
Essential Duties and Responsibilities: include the following.
Other duties may be assigned.

-Conducts a psycho/social assessment on all new clients; conducts additional follow-up assessments as required by regulation or deemed necessary by the healthcare team.
-Provides crisis intervention counseling.
-Provides short term therapy as needed for individuals, couples and/or families.
-Assists the patient in prioritizing needs and coordinating referrals.
-Advises, counsels and, when appropriate, assists the patient's support system in problem identification, analysis, suggestions for interventions and follow up plans.
-Develops and tracks mental health treatment plans.
-Reports suspicions of child and/or adult abuse to the appropriate authorities according to statutes in collaboration with the Director of Psychiatry, the patient's medical provider and AHF's General Counsel.
-Refers clients to the appropriate program or resource, and when required, assists clients in the enrollment process to access benefits.
-Maintains a log of referrals and referral follow-up.
-Collaborates with the healthcare center team (nursing, medicine, benefits) to link client with community services and programs as required by the treatment plan.
-Is a member of and attends the Mental Health Team conference meetings.
-Maintains an active client case load in accordance with productivity standards determined by AHF management.
-Reviews cases on an on going basis to assure maintenance of accurate information,benefits and appropriateness of the treatment plan.
-Utilizes a standardized format for documentation and maintains complete and accurate files on all clients; documents response to the treatment plan and documents all services provided.
-Conducts continuous quality improvement activities to assure caliber of service.
-Serves as agency liaison to other service and community organizations as assigned.
-Participates in training and consultation on HIV mental health issues to healthcare center staff, clients and the general public as assigned.
-Prepares monthly reports for the Office of AIDS Programs and Policies (OAPP) as required.
-Prepares quarterly reports for Positive Healthcare.

AHF Meetings:
Mental Health Team conference meetings

Other Qualifications:
-Minimum of one year of inpatient healthcare experience desired.
-Two years of clinical work, preferably with emphasis on chronically ill patients.
-Must be able to travel 20% of the time.
-Strong organizational and record keeping skills.
-Refined interview techniques.
-Knowledge of HIV/AIDS related issues and concerns as they relate to the diverse populations.
-Familiarity with organic and behavior disorders, dementia,
crisis intervention techniques and social/family systems.

Other Skills and Abilities:
-Bilingual (English/Spanish) skills strongly preferred.
-Strong interpersonal and communication skills.
-Ability to work as a team player.

Education and/or Experience:
-Master's degree (MSW) in Social Work or equivalent from an accredited school of Social Work.

Computer Skills:
To perform this job successfully, an individual should have knowledge of MS Office Word Processing software.

-Valid California Driver's License.
-Proof of liability insurance.
