Position 
Temporary Full – Time Transition-Age Foster Youth Advocate, Children’s Rights Project

Duration of Position:

Project Description
Through a combination of education advocacy, legal representation, transition support, and case management, the Transition-Age  Foster Youth (TAFY) program strives to ensure that foster youth in grades 9-12 graduate from high school, have access to post-secondary education options, and receive the necessary resources to achieve success. 

Job Description

The Foster Youth Advocate works closely with a Staff Attorney partner and with the TAFY team, and has the following general responsibilities: 
  • Conduct outreach, intake and assessment with foster youth
  • Advocate for individualized and effective transition and education plans with foster youth and monitor the progress of foster youth
  • Provide case management to foster youth with a specific focus on grade promotion, enrollment in the “A-G” curriculum and access to post-secondary education in the form of two-year or four-year colleges;
  • Work collaboratively with foster youth’s Children’s Law Center attorneys, caretakers, social workers and others involved in providing support to the youth;
  • Provide foster youth and their caretakers with “Know Your Rights” trainings on education and transition related issues for foster youth;
  • Conduct outreach and training presentations to school staff, community based agencies and other stakeholders working with transition-age foster youth ;
  • Meet regularly with the supervising Staff Attorney and with the TAFY team to identify case specific and systemic issues that require legal interventions;
  • Develop and maintain complete and accurate case files and notes for all youth; obtain and review all relevant education, mental health, health and other records; enter case information into computerized case management system; and work with TAFY team to collect, analyze, and report on outcome data.
Skills Required/Preferred: 
  • A Masters Degree in Social Work (MSW) or equivalent experience;
  • Experience working with high school aged youth;
  • An interest and/or experience in juvenile court proceedings, including the dependency and delinquency systems;
  • Knowledge of the special education, college access, and transition planning processes and experience with advocacy in these areas;
  • Excellent interpersonal, written, advocacy and phone skills;
  • Public speaking experience;
  • Proficiency in the Spanish language (preferred).

Juvenile Case Manager Position
JOB ANNOUNCEMENT
The mission of Centinela Youth Services (CYS) is to strengthen families and communities by
empowering youth to resolve conflicts and overcome obstacles in order to become successful students
and contributing adults. Centinela Youth Services (CYS) is a 501c3 non-profit, restorative justice
organization established in 1975 to provide mediation and conflict resolution services to at-risk youth.
This position will be assigned to The Everychild Restorative Justice Center (The Everychild Center),
which is co-located at the South Bay One-Stop Business & Career Center in Inglewood. The Every child Center is a new service established by CYS to provide evidence-based mediation services linked to intensive case management and wraparound community-based services to significantly reduce the number of teens attached to the Inglewood Juvenile Court and other agencies in the L.A. County juvenile justice system.

Salary & Benefits: This is a part-time position (up to 30 hours/week) paying an hourly wage from $17.38 to $21.84.

Minimum Qualifications: Requires a Bachelor’s Degree in social work, counseling, sociology,
psychology or closely related field AND two years of case management experience in social work
and/or juvenile justice or a closely related field. Additionally, strong interpersonal skills and an ability
to communicate effectively with youth of all cultural and socio-economic backgrounds is required as
well as analytical, teamwork and computer skills in MS Word and Excel. Must have reliable
transportation, auto insurance and a valid California Driver’s License.

Duties Include:
Coordinate with juvenile justice agencies for referral of cases from courts, district attorneys, local
law enforcement, probation and school discipline;
Conduct in-depth assessments with youth and families to determine participant needs, strengths and
risk factors including educational needs; probationary needs; and healthcare, welfare and mental
health needs;
Monitor participants to ensure compliance with court orders that may include curfew, community
service or special classes;
Refer participants to service providers for social and developmental services such as victim/offender
and family mediation, mental health treatment and/or counseling, education rights assistance,
academic assistance, life skills, and vocational/job training;
Maintain regular contact with providers to monitor progress;
Make home and school visitations as needed;
Assist with the development of effective assessment and case management procedures and systems;
Maintain accurate case files; collect and report data as needed for reports and evaluation;
Participate in staff training and partner meetings; and
Other duties as assigned.

Job Title:       Quality Assurance Clinician

SUMMARY
The Quality Assurance Clinician will provide review and oversight of all Medi-Cal documentation, client records, and compliance requirements to ensure that Lamp’s Wellness Center operations meet or exceed required documentation and compliance standards for the Los Angeles County Department of Mental Health. The Clinician will also provide staff consultation and trainings as applicable to meeting documentation, compliance standards, and quality assurance best practices.

ESSENTIAL DUTIES & RESPONSIBILITIES
1.     Provide detailed and accurate review of all DMH Medi-Cal documentation including weekly progress notes, Adult Initial Assessments, Annual Assessment Updates, Client Care Coordination Plans, Discharge Summaries, Progress Notes, and all other clinical and administrative forms associated with each client record; co-sign documentation and collect signatures for documentation as needed
2.     Coordinate with Wellness Center Associate Director to ensure compliance with documentation standards and completion of required documentation
3.     Provide Medi-Cal documentation training to all Wellness Center staff
4.     Audit Wellness Center charts to ensure presence of required documentation, and provide clinical review
5.     Coordinate with Director of Service Delivery and Chief Operating Officer regarding quality and compliance issues
6.     Liaison with Los Angeles County Auditor Controller during Medi-Cal audits
7.     Complete Department of Mental Health Adult Initial Assessments, Annual Assessment Updates, Discharge Summaries, Client Care and Coordination Plans, and Progress Notes as needed


Requirements
1.     Active LCSW required
2.     Two years full-time, post-graduate experience working with Medi-Cal/DMH documentation, including both direct practice and review
3.     Highly detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
4.     Strong training, project management, and team-building skills
5.     Able to behave with sensitivity, tact, and responsiveness
6.     Outstanding clinical review skills
7.     Proficient in Microsoft Word, Outlook, Excel, and database applications
8.     Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
9.     Contract and/or quality assurance experience preferred

WORK ENVIRONMENT
1.     Field (may need to travel) and indoor office environment
2.     Will necessitate working in busy and loud environments
3.     Will be exposed to elements like cold, heat, dust, noise and odor
