Case Manager - Pico Youth and Family Center

Primary Responsibilities

The primary responsibilities of this position are establishing relationships with youth, providing case management, conducting street outreach, and maintaining a safe environment in the center. This includes assessing situations, developing individualized goals and plans, and providing appropriate referrals. One on one goal setting and tracking of all case managed youth through an outlined achievement plan centered around academics, employment, family, community and personal goals.  Integral to this position is the ability to motivate peek performance from at-risk, gang involved and probationary youth. The Case Manager must build and maintain working relationships with clients for youth serving programs and agencies in the community

The person in this position must be able to perform the duties outlined on this job description. A valid driver's license and current automobile insurance is required. Night driving may be necessary. Must be able to work under stress and be able to respond to emergencies. Mobility is required. Ability to prepare documentation and communicate reports. Evening hours, rotating Saturdays, and some holidays. In addition, must have set field hours and set office hours and must comply with these hours. Demonstrate a high level of respect for youth as individuals and as part of a community and culture, and an understanding of different cultural practices.

Responsible for building relationships with at-risk youth and their parents, to assist in their empowerment.  Encourages youth to give back to their community and to become leaders in their neighborhoods.  Provide intensive case management to a minimum of  25 cases per year. Participates as a member of the management/counseling team. Must also be able to perform any other duties provided by Executive Director and On-Site therapist.
·Interviews participants to assess their needs and goals.
·Performs comprehensive assessments on individual clients, develops and implements service plans, identifies programs, and involves client’s family when appropriate.
·Creates awareness of problems and situations that participants may not view as dangerous.
·Works as an advocate for the participant once contact has been established with the pertinent programs.
·Monitors the participant’s individual goals and motivates new goals.
·Creates and updates accurate files and maintains complete client chart notes which will reflect the progress of participants.
·Provides information and assistance, referral and advocacy on behalf of clients. And follows-up with service providers.
·Conduct outrach to attract new participants.

 Minimum Qualifications
Bachelor’s degree in Social Science and two year’s experience working in the field.
Must be Fluent in English/bilingual if applicable.
Must have a car, valid California driver’s license, and show evidence of insurability.
Must have excellent oral, written and organizational communication skills.
Must be effective in conflict resolution.
Must have computer skills, specifically in Excel and Word.
Must have good attention to detail

PROJECT: ACCESS AND QUALITY HEALTHCARE PROJECT

Organization Summary
Community Health Councils, Inc. (CHC, Inc.), is a community-based, non-profit organization dedicated to increasing access to and the quality of health care for uninsured, underinsured, and under-resourced communities in Los Angeles and California. Our work is founded upon coalition-building, community partnership, and collaboration across multiple sectors.

Project Summary
The Access to Quality Healthcare Project at CHC works through local coalitions to eliminate health disparities, increase the number and quality of healthcare resources, and achieve health equity in South Los Angeles and LA County. The project’s specific objectives aim to: (1) build a comprehensive and sustainable system of care for South Los Angeles; and (2) Increase the knowledge and participation, and leadership role of consumers in policy and local resource development to address health disparities.

Research Associate Summary
The Research Associate is responsible for providing critical research, planning, analysis and administrative support for the South LA Healthcare Leadership Roundtable. The Roundtable coalition is comprised of top-level executives and officers working to build an integrated delivery system of care in South LA. This position requires a high level of strategic thinking, communication, leadership, and the ability to work and connect with community members, community-based organizations, public administrators and elected officials, researchers, and more. An in-depth knowledge of the safety net healthcare system in South LA is preferred. The RA is responsible for conducting examinations and study into specific areas of healthcare delivery, i.e. public and private financing mechanisms, public policies and procedures, access to care, current regulations and legislation, infrastructure analysis, and more. Under the direction of the Policy Director and the Executive Director, the RA will also assist in developing the Roundtable’s annual policy and project priorities, goals and objectives, and outreaching to appropriate organizations regarding participation and/or membership. Overall, the RA is also responsible for meeting the annual agency objectives, contributing to the mission and goals of Access and Quality policy area and CHC as an organization.

Description

  • The Research Associate’s responsibilities will include, but are not limited to:
  • Data collection, management and analysis
  • Retrieve, organize, synthesize, and produce information from a variety of mediums
  • Assist with management of coalition tasks including, but not limited to: facilitating coalition meetings, outreach to new organizations to join coalitions and campaign activities, and serving as a liaison between the coalitions, elected officials and other decision-makers, and representatives of the major stakeholder organizations. 
  • Provide administrative support such as coordinating meeting logistics, including notices, minutes, photocopying and collating relevant materials, food, room reservations, informational updates, and more.
  • Provide program and administrative support in the following areas: program planning and implementation; project deliverables development; project tracking to ensure deadlines are met and project deliverables are completed on time; and anticipate and problem-solve for unforeseen project issues.
  • Coordinate monthly coalition meetings, workgroups, and any other project activities as needed.
  • Effectively communicate with coalition members and decision-makers regarding coalition work including: any policy recommendations, advocacy efforts, community-based issues, and any other work that advances the mission and goals of CHC.
  • Manage the coalition campaign and member databases.
  • Research DHS and relevant policy implementation and impact by using on-line resources, attending meetings, contacting DHS staff, and providing summary reports to management and coalition members.
  • Attend and represent the coalition and CHC at all relevant external meetings, conferences, trainings, etc.
  • Assist with other tasks as requested by the Policy Director.

Qualifications/Experiences:

  • Bachelor’s degree in health related field is required, Masters’ degree preferred.
  • Minimum of 2-3 years of community-based experience including familiarity with coalition or meeting facilitation and health care issues and/or community-based organizations.
  • Ability to interact with diverse communities, legislatures and government, etc.
  • Experience working in underserved and minority communities, particularly the South LA region.
  • Ability to multi-task in a fast-paced environment.
  • Willingness to travel throughout the local area.
  • Must possess excellent oral and written communication skills.
  • Highly organized.
  • Proficient in Microsoft Office and Internet.
  • Bilingual – Spanish/English (preferred).

Salary is DOE. This position is classified as exempt.

TITLE:           Father Engagement Training Specialist
HOURS:        Full Time
STATUS:       Exempt

JOB OVERVIEW:     This position is for an experienced trainer or consultant who has knowledge of father engagement strategies and family strengthening practice and/or family resources centers.  Experience in working with fathers preferred. Familiarity with strength based approaches, organizational sustainability, and case management is desired.  This position will focus on the Father Engagement Project within Strategies, which is a research based approach to engaging fathers in order to achieve positive outcomes for children. The person in this position will provide technical assistance and training to family strengthening organizations and network around agency outreach, recruitment, engagement, and retention.  Experience with Internet technologies, website development and maintenance and significant experience with Microsoft Office Suite all preferred.  Extensive traveling in California is required.  Reports to the Regional Director and Father Engagement Project Manager.

DUTIES: 
1.       Provide regional and statewide promotion, training and technical assistance to agencies and networks involved with the Father Engagement Project.
2.       Develop and maintain strong working relationships with potential and engaged Father Engagement sites throughout California, utilizing a variety of approaches including site visits, e-mail and telephone contact.
3.       Maintain and develop content for the web page in order to promote a learning community.
4.       Participate in Father Engagement Project conference planning, training and workshop development.
5.       Promote general as well as specific benefits, and systemic solutions to recruitment, engagement, retention of fathers as a resource to family strengthening
6.       Track and respond to technical assistance requests via fax, telephone and e-mail
7.       Work with Strategies team throughout the state in meeting goals of Strategies Father Engagement related scope of work.
8.       Contribute to the developing strategic planning to increase resources in support of the Father Engagement Project.
9.       Other duties as assigned.

QUALIFICATIONS:            Requires demonstrated experience in social service field with strong computer and internet skills, as well as written and verbal communication skills. Training experience – specific to adult learning principles - is required, including the ability to work with groups such as front-line staff, community members and management staff.  Ability to handle complex tasks, and relate to a wide range of professionals is a must.  Demonstrated knowledge & experience of specific social service programs falling into the category of family support, family resource, or family strengthening models.  Must have the ability to work independently while also comfortable working in a group setting.  Ability to travel extensively is required. Masters level education, or equivalent experience, is preferred.

Part-Time Work at a Dialysis Facility


I’m seeking a (new) MSW that could work directly with me at my dialysis facility at the corner of La Cienega and the 10 fwy.  I will provide all training.  

I need someone one day a week (8 hours, preferably early morning Saturdays so can still have a decent weekend but I’m very flexible in terms of days/time) to start with but with possibility of growing into more.  Span spkg (intermediate is fine).

This is medical social work and case management, but sharp clinical skills are necessary.  Start part-time and grow.  I need a serious, dedicated SW who is really into it.  I need a social work practitioner who strives for excellence and believes in providing world-class service as taught in grad school.

I need someone from an excellent MSW program such as UCLA b/c I demand excellence in my work and who will work with very complex cases with care and compassion.  The current healthcare system and economy have made it brutal for these chronic patients in terms of resources (financial, medical, concrete, clinical).  

Lots of face-time with patients (assessment, education, counseling) and lots of accompanying documentation.  Lots of education delivery and community resource linkage; lots of emotional support.  This clinic is demanding, stress-filled, noisy, busy and very tough – challenging to the max.
