About the Coalition for Responsible Community Development (CRCD)
Founded in 2005, CRCD’s mission is to change lives and build neighborhoods responsibly. We work
to better sustain, coordinate, and improve local planning, development, and community services that
address the needs of low-income residents and small businesses in South Los Angeles. CRCD serves a
15 square-mile area immediately south of downtown Los Angeles. The Vernon-Central neighborhood
is generally located east of the 110 Harbor FWY to Alameda, and south of Washington Boulevard to
Slauson, in the southern portion of Los Angeles City Council District 9.

Summary of Responsibilities
Responsible for coordinating supportive services for formerly homeless individuals diagnosed with
a mental illness. On-site and off-site services shall be delivered to tenants who reside at multi-unit
permanent supportive housing complex. Linkages to services include but not limited to intensive case
management oversight, life skills workshops, money management, educational attainment, access to
health services, counseling, job training and placement opportunities, and resident activity coordination
just to name a few. This position requires extensive coordination within multiple CRCD Departments,
Property Management, Department of Mental Health (DMH), Los Angeles County Social Service
departments and South Los Angeles community organizations, to ensure housing stability and achieve
optimum outcomes for tenants at this building.

Specific Duties
1. Work with each tenants’ case manager to conduct initial housing intake and needs assessment
for each tenant assessing service needs.
2. Assist tenants in determining personal goals and plans to achieve such goals through the
development of the Individual Service Plan (ISP).
3. Conduct a minimum of one face-to-face contact with each tenant on a bi-monthly basis.
4. Conduct quarterly meetings with a designated representative of the primary service provider for
each tenant to discuss tenant’s progress.
5. Work with case manager to update the tenant’s service plan annually.
6. Act as a liaison among the Project Manager, the property management company, the tenant
and the primary mental health provider to facilitate the communication of concerns that could
jeopardize the tenant’s residency.
7. Provide information and instruction to clients regarding Fair Housing, tenant rights and
responsibilities, including observation of rental agreement and property rules.
8. Act as a liaison and coordinate linkage to services with numerous public social service agencies
and community providers, three categories in particular will assist them in achieving and
sustaining independence: employment and education/ training activities; money management
counseling; and legal and benefits counseling.
9. Assist clients in obtaining rental assistance: move-in and eviction prevention services.
10. Address tenant grievances, mediate resident disputes, and resolve any issues with property
management that might affect the client’s tenancy.
11. Enforce housing rules in collaboration with property management.
12. Development of workshop opportunities on relevant topics including housekeeping, money
management, parenting, daily living skills (including transportation, training in self advocacy/
community accessibility) needed to maintain housing and achieve independence.
13. Data collection/entry into database to assemble reports and evaluate program.
14. Organization and management of client files.
15. Monitor status of tenants who have moved into housing and prepare tenants who are deemed
ready to transition to mainstream non-supportive housing.
16. Comply with the necessary documentation and reporting requirements of the State Department
of Mental Health (SDMH) and DMH
17. Compile and submit monthly progress reports to DMH that track tenants’ residency status and
the supportive services which have been provided to them.

MINIMUM QUALIFICATONS

• EXPERIENCE:
Position requires a minimum of five years experience delivering psychosocial rehab related
services to individuals diagnosed with a mental illness. Experience obtained in Los Angeles
County public social services and/or housing related services is greatly desired.

•KNOWLEDGE AND SKILLS:
Knowledge of community resources, especially related to the Mental Health, Education, Medical,
and Social Services fields, and Workforce Development
Knowledge of housing assistance programs
Tenant/landlord law and tenant/landlord/relations
Advocacy and negotiation skills
Creative problem-solving skills
Strong communication and interpersonal skills
Ability to work in a team and independently
Coordinating collaborative partnerships
Proven strong ability to navigate Los Angeles County public social services.
Data entry and administrative reporting
Must have valid CA driver’s license, as driving vehicle to various community meetings is required
Flexible work schedule to include some evenings or weekends, and on call cell phone services
when necessary

• EDUCATION:
Bachelor’s Degree in social science field or related field preferred. Additional work experience
may be substituted for education

Quality Assurance Clinician
Job Description

Job Title:        Quality Assurance Clinician
Dept:               Wellness Center
Status:             Half-time/Exempt

SUMMARY
The Quality Assurance Clinician will provide review and oversight of Medi-Cal documentation and client records to ensure Lamp operates a high-quality Wellness Center that meets or exceeds DMH and Medi-Cal standards for documentation.  The Clinician will also provide Wellness Center staff consultation and trainings as it applies to meeting Medi-Cal and DMH documentation standards.

ESSENTIAL DUTIES & RESPONSIBILITIES
            Review all Medi-Cal documentation including weekly progress notes, Adult Initial Assessments, Annual Assessment Updates, Client Care Coordination Plans, Discharge Summaries, and all other clinical and administrative forms associated with the client record
            Track and monitor documentation due dates and annual cycle dates
            Track and reconcile SFPR function on paperwork, and collect signatures for documentation when needed
            Audit charts and reconcile findings to county and state requirements and ensure presence of required documentation
            Assists in the development and drafting of policies and procedures for the Quality Assurance function
            Gathers data and completes and submits monthly reports on quality assurance activities
            Identifies need for and provides documentation trainings
            Attends Los Angeles County Department of Mental Health Quality Improvement Committee for respective service provider area to maintain a working knowledge of requirements

Requirements
            LCSW or MSW (Supervision hours are not available for this position.)
            One year full-time, post-graduate experience working with Medi-Cal/DMH documentation, including both direct practice and review
            Leadership and training skills to effect change where needed in documentation functions.
            Ability to display sensitivity, tact, and responsiveness in various situations and needs.
            Outstanding clinical review and strong team-building skills
            Proficient in Microsoft Word, Outlook, Excel, and database applications
            Highly detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
            Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision


WORK ENVIRONMENT
            Field ( may need to travel) and indoor office environment
            Will necessitate working in busy and loud environments
            Will be exposed to elements like cold, heat, dust, noise and odor

EISNER PEDIATRIC & FAMILY MEDICAL CENTER



Integrated Behavioral Health Program Coordinator        

Eisner Pediatric & Family Medical Center is a Community Health Center in downtown Los Angeles serving both the primary care medical and mental health needs of our community.  We currently have an opening for a bilingual (Spanish) LCSW to coordinate Eisner’s Integrated Behavioral Health Program.  This program consists of two parts: one, an innovative, cutting-edge program that provides behavioral health services to our patients by co-locating therapists in our medical departments and working closely with the physicians. The coordinator will have an opportunity to supervise MSW students in this program The other program is in coordinating our HealthyWayLA program which serves our adult population and is a collaboration between the physicians and clinicians.. The candidate must be highly flexible, able to work in a fast-paced atmosphere, and collaborative.  Our population is primarily low-income Latino patients who suffer from anxiety and depression and serious psychosocial factors.  This program does not serve the chronically mental ill.

Qualified candidates would have the following:

Licensed in the state of California as an LCSW
Bilingual in Spanish
Two years clinical supervisory experience
Experience working with diverse populations
Experience working with adults in the mental health system
Ability to utilize short-term, functional interventions conducive to providing integrated care.
Experience with an Integrated Behavioral Health Program highly desirable.
Experience with DMH paperwork desirable
Training in Problem-Solving Therapy desirable

Pico Youth and Family Center


Case Manager - Pico Youth and Family Center

Primary Responsibilities

The primary responsibilities of this position are establishing relationships with youth, providing case management, conducting street outreach, and maintaining a safe environment in the center. This includes assessing situations, developing individualized goals and plans, and providing appropriate referrals. One on one goal setting and tracking of all case managed youth through an outlined achievement plan centered around academics, employment, family, community and personal goals.  Integral to this position is the ability to motivate peek performance from at-risk, gang involved and probationary youth. The Case Manager must build and maintain working relationships with clients for youth serving programs and agencies in the community

The person in this position must be able to perform the duties outlined on this job description. A valid driver's license and current automobile insurance is required. Night driving may be necessary. Must be able to work under stress and be able to respond to emergencies. Mobility is required. Ability to prepare documentation and communicate reports. Evening hours, rotating Saturdays, and some holidays. In addition, must have set field hours and set office hours and must comply with these hours. Demonstrate a high level of respect for youth as individuals and as part of a community and culture, and an understanding of different cultural practices.

Responsible for building relationships with at-risk youth and their parents, to assist in their empowerment.  Encourages youth to give back to their community and to become leaders in their neighborhoods.  Provide intensive case management to a minimum of  25 cases per year. Participates as a member of the management/counseling team. Must also be able to perform any other duties provided by Executive Director and On-Site therapist.
·Interviews participants to assess their needs and goals.
·Performs comprehensive assessments on individual clients, develops and implements service plans, identifies programs, and involves client’s family when appropriate.
·Creates awareness of problems and situations that participants may not view as dangerous.
·Works as an advocate for the participant once contact has been established with the pertinent programs.
·Monitors the participant’s individual goals and motivates new goals.
·Creates and updates accurate files and maintains complete client chart notes which will reflect the progress of participants.
·Provides information and assistance, referral and advocacy on behalf of clients. And follows-up with service providers.
·Conduct outrach to attract new participants.

 Minimum Qualifications
Bachelor’s degree in Social Science and two year’s experience working in the field.
Must be Fluent in English/bilingual if applicable.
Must have a car, valid California driver’s license, and show evidence of insurability.
Must have excellent oral, written and organizational communication skills.
Must be effective in conflict resolution.
Must have computer skills, specifically in Excel and Word.
Must have good attention to detail
