Please join us in a FREE day long educational opportunity to learn about recent issues impacting the Latino/a community in the Los Angeles area.  This is a unique event that will address topics through a social work and social justice perspective.  Students and professionals interested in dialoguing about these complex issues are invited to come and learn culturally sensitive interventions to better serve our community.


The conference will include workshops on Shared Communities (Black & Brown) | Juvenile Justice | Domestic Violence | Gangs | PTSD in the Immigrant Community | Mental Health | LGBTQ | Community Organizing for Violence Prevention.  You will also have the chance to network with experienced practitioners, professionals, students, and community members at a networking reception following the conference.  Lunch will be provided.

LAʼs Promise (formerly MLA Partner Schools) Opportunity

ASSOCIATE DIRECTOR OF HEALTH INITIATIVES

LAʼs Promise (formerly MLA Partner Schools) is a non-profit organization that is radically shifting the
education, health and social outcomes for thousands of youth in one South Los Angeles community.
Within an area we call “LAʼs Promise Neighborhood,” LAʼs Promise transforms chronically failing public
schools, opens new schools and remakes schools into community hubs that offer comprehensive
support services for students and families.

LAʼs Promise is seeking an Associate Director of Health Initiatives to design and implement high
impact health initiatives for 8,000 students and their families. He/she will work under limited
supervision with considerable latitude and independent judgement. This person will be tasked with
working with various stakeholders including parents, teachers and health partners to develop
exceptional health programs that are integrated into the school day and radically shift the health
outcomes for thousands of youth.

Primary Responsibilities
• Execute the organizationʼs universal health screening program.
o Ensure high student participation through various outreach activities such as classroom
presentations, parent meetings and community health fairs.
o Collaborate with the schoolʼs health provider, St. Johns Family Well Child Center, to
develop an efficient system for implementing student screenings. Manage all logistics
including collecting consent forms, maintaining records and scheduling screenings.
o Develop a health intervention program for students with high health needs (Program
would include components such as fitness activities, nutrition education, healthy cooking
courses and parent education classes).
• Develop a plan to improve health and physical education (PE) courses across all school sites.
o Collaborate with LAʼs Promiseʼs academic team and the schoolsʼ health and PE
teachers to augment health and PE curriculum. Curriculum will be aligned to California
State standards but will feature non-traditional activities such as yoga and kickboxing
during PE classes.
o Support the development of the health and PE departmentʼs professional development
program. Provide ongoing support to teachers to ensure the successful implementation
of curriculum.
• Work under the direction of the VP of Community and Health Services to develop and
implement other high impact health initiatives.
o Research successful school based health models that have been proven to improve
student health outcomes.
o Expand the scope of universal health screenings to include all schools and to
incorporate a mental health screening.
o Develop innovative, high value community health projects (ex. social enterprise project,
community garden, school lunch program) within LAʼs Promise Neighborhood. Develop
detailed work plans, manage milestones/deliverables and drive overall project to a
successful completion.
• Other duties as assigned

Required Experience
• At least 5 years experience as a health advocate/leader in an underserved community.
• Strong organizational, administrative, verbal and written communication skills
• Strong project management skills and background
• Resourceful, creative, able to multitask, prioritize and manage time effectively
• A strong collaborator, good listener and strategist; comfortable receiving input from many
sources, and able to analyze and formulate disparate information into a sound, well-organized
plan
• Entrepreneurial - a self starter with a high energy level, an action-oriented individual who
excels with limited supervision
• BA/BS required; Advanced degree in health related field preferred
• Spanish language skills a plus

This is a permanent full-time immediate opening in Los Angeles, CA. Competitive compensation

Homeless Health Care Los Angeles (HHCLA) Opportunity

Homeless Health Care Los Angeles (HHCLA), founded in 1985, provides comprehensive case management services for homeless people with health-related problems and provides community-based education to homeless service providers. Our mission is to work towards the optimal health of homeless people through direct services, education and advocacy.

Job description: The primary purpose of Homeless Health Care Los Angeles Access to Housing for Health project is to provide permanent, affordable housing linked to appropriate services for homeless individuals, who are frequent users of the Department of Health Services. The clinical social worker works directly with program participants and develops, arranges and coordinates treatment services. The clinical social worker works to improve the health of homeless individuals by assisting them with the provision of stable housing, development of a comprehensive treatment plan and facilitating linkages to medical care, mental health care, alcohol/substance use treatment and other supportive services. Once the individual has obtained permanent housing, the clinical social worker assists them in maintaining their housing.

Essential Functions:
1. Conduct comprehensive assessments for case management service needs.
2. Develop individualized treatment plans with measurable goals and objectives and assist
participants in achieving their desired outcomes.
3. Coordinate, prepare and maintain required charting and documentation in a timely and thorough
manner.
4. Participate in daily debrief sessions, weekly supervision, department and agency-wide
meetings, training and development opportunities.
5. Develop relationships with representatives in other agencies to support individuals in attaining
an array of community-based services.
6. Maintain familiarity with HHCLA policies and procedures and other applicable local, state and
federal regulations.
7. Assist in identifying and securing resources for participants.
8. Maintain client records according to the HIPPA Act of 1996 and adhere to all client
confidentiality requirements and standards.
9. Participate in the overall evaluation of program services and data collection for reporting by
maintaining accurate information in program participant files.
10. Perform home visits within scheduled time frame, asses for needs, and refer clients to services within their community.


Qualifications: Master’s Degree in Social Work/Welfare required with a minimum of two years experience in
working with culturally diverse populations. Experience with homeless and low-income populations
preferred. Familiarity with the principles of harm reduction, mental health and the healthcare delivery
systems in Los Angeles County required. Strong written and oral communication skills required.
Computer skills necessary to perform job duties required. Able to stand, walk and sit for the purpose
of performing the essential job functions. Qualifications may be subject to modification, based upon
the Americans with Disabilities Act. Must possess a valid California Driver’s License, properly
registered vehicle and liability insurance. Bilingual/bicultural preferred.
