Project Coordinator, (Assistant Project Manager) will oversee the implementation of the Long Beach Alliance for Children with Asthma (LBACA’s) Asthma Case Health Worker Program.  Works as a member of the Long Beach Alliance for Children with Asthma to advocate for children with asthma and their families through the supervision of outreach and education activities by the Community Health Workers (CHWs).  All activities will be supervised by the Project Manager.

Essential Job Duties:

Connects with clinical and non-clinical sites to participate in the program (including non-profit organizations, schools, clinics, and hospitals in the target areas of Wilmington, Carson and Long Beach).
Also collaborates with other CHW programs on training.
Connects with provider offices and community health worker programs in the target area to strengthen partnerships with current collaborations.
Supervises the activities of the five CHWs, including assigning outreach sites and clients to CHWs, monitoring home and provider visits, group sessions, chart review, and data collection and management practices.
Obtains and manages project materials and resource information including meeting minutes, data entry, project reports, reference materials, resource files, and client and provider site files/contact information.
Develops community awareness of the CHWs program purposes, goals, and objectives.
Bilingual English/Spanish speaking literacy (reading, writing and
speaking) is a plus but not required.
This is a Full Time position with some work on weekends.
Minimum Requirements/Experience/Education:

Proven ability to develop respectful working environments within ethnically and linguistically diverse communities.
Previous experience coordinating community projects or programs for improved health outcomes.

Previous experience in resource development and management.
Proven time management skills and flexibility in the workplace.
Must have excellent computer skills including Microsoft Office (Word, PowerPoint, Excel and Access), as well as proficiency using Internet Explorer.
Prefer to have some grant writing/reporting experience to assist the Project Manager.
A Bachelor’s degree or equivalent experience in Communication, Health Science, Social Work, Public Health or a related field.

Requires the use of a vehicle; must have a current California Drivers License, and proof of auto insurance.
