Job Summary:
Responsible for providing individual, family, and group mental health services to clients in the Full Service Partnership (FSP).
Major Responsibilities
  1. Provide individual, family and group therapy to assigned caseload.
  2. Serve as care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
  3. Provide “whatever it takes” services as defined by treatment model and treatment team to consumers including providing services where the consumer lives, assisting with housing, benefits, employment, education, transportation.
  4. Assist client in developing independent living skills to promote independence and self-sufficiency
  5. Co-facilitate and lead psycho-social rehabilitation groups.
  6. Provide case management services as needed.         
  7. Develop outpatient mental health services that meet the needs of the clients.
  8. Provide outreach services to individuals within this community.
  9. Participate in milieu activities of FSP.
  10. Work as part of treatment team in collaboration with the psychiatrist, therapists, case manager and other team members.
  11. Assist in crisis and symptom management within FSP programs.
  12. Provide a minimum of 65% field based services.
  13. Work as an active FSP team member and closely collaborate with team members.
  14. Work as part of treatment team in collaboration with the psychiatrist, therapists, case manager and other team members.
  15. Develop effective individualized treatment plans and monitor consumer progress.
  16. Develop services to meet the needs of consumers.
  17. Provide 24 hour, 7 days per week after hours coverage when assigned.
  18. Maintain documentation in compliance with agency, HIPAA, and DMH standards.
  19. Assist in ongoing maintenance of consumers’ charts and other related documentation.
  20. Complete all clinical documentation in a timely and accurate manner.  
  21. Comply with the agency’s policies and procedures.
  22. Arrange work schedule in accordance with the agency’s needs.
  23. Attend regular staff meetings and other meetings as assigned.
  24. Comply with the agency’s policies and procedures.
  25. Meet expected performance hours as assigned by supervisor.
  26. Perform other duties as assigned.
Minimum Requirements:
MA, MSW or Ph.D/Psy.D  from regionally accredited graduate program. Two years minimum clinical experience in delivering mental health services. Some experience working with persistently mentally ill adults and families preferred. Knowledge of co-occurring disorders and trauma preferred. Valid CA driver’s license and current automobile insurance.
