TEMPORARY Case Manager

We are seeking an experienced individual to provide case management and support for women at risk of having poor birth outcomes as part of an initiative funded through First Five LA Best Babies Collaborative.

Hours/Salary:  Full time, temporary; November 2011 thru April 2012

$44K-$48K/year; position may require evening and/or weekends

Duties/Responsibilities including but not limited to:

·           Provides case management, service coordination and resource linkage for women at risk of having poor birth outcomes

·           Ensures the provision of comprehensive services to participants based on professional assessment  of psychosocial status

·           Makes home visits to address issues related to promoting healthy pregnancy, post-partum and inter-conception care and assist with daily life management for at-risk women

·           Attend all Best Babies Collaborative meetings and workgroups and provide feedback and updates to Friends of the Family staff

·           Maintain timely and accurate case files and provide necessary data to Best Babies Collaborative for data entry

Education/Qualifications/Skills:

·           Master Degree in Social Work or Counseling and three years case management experience in related field

·           Fluent in oral and written English & Spanish, required

·           Excellent oral and written communication skills required (please submit a writing sample with your resume)

·           Experience in home visits preferred

·           Expert knowledge of quality prenatal, postpartum and interception care behaviors, child and family development principles, helpful

·           Demonstrated knowledge of community resources & social services

·           Knowledge of dynamics of at risk populations

·           Problem solving and crisis intervention skills

·           Ability to work independently and as part of a team

·           Proficiency in Word, Excel, Power Point, Access, Publisher & Outlook

·           Must have a car, be able to drive, have a valid CA driver’s license, and current auto insurance

·           Satisfactory completion of all background screening requirements
