AIDS Healthcare Foundation Position

We are searching for a Medical Social Worker to work in our
Healthcare Center located in Hollywood California. The hours are Monday-Friday from 8:30 a.m. to 5:30 p.m.
Essential Duties and Responsibilities: include the following.
Other duties may be assigned.

-Conducts a psycho/social assessment on all new clients; conducts additional follow-up assessments as required by regulation or deemed necessary by the healthcare team.
-Provides crisis intervention counseling.
-Provides short term therapy as needed for individuals, couples and/or families.
-Assists the patient in prioritizing needs and coordinating referrals.
-Advises, counsels and, when appropriate, assists the patient's support system in problem identification, analysis, suggestions for interventions and follow up plans.
-Develops and tracks mental health treatment plans.
-Reports suspicions of child and/or adult abuse to the appropriate authorities according to statutes in collaboration with the Director of Psychiatry, the patient's medical provider and AHF's General Counsel.
-Refers clients to the appropriate program or resource, and when required, assists clients in the enrollment process to access benefits.
-Maintains a log of referrals and referral follow-up.
-Collaborates with the healthcare center team (nursing, medicine, benefits) to link client with community services and programs as required by the treatment plan.
-Is a member of and attends the Mental Health Team conference meetings.
-Maintains an active client case load in accordance with productivity standards determined by AHF management.
-Reviews cases on an on going basis to assure maintenance of accurate information,benefits and appropriateness of the treatment plan.
-Utilizes a standardized format for documentation and maintains complete and accurate files on all clients; documents response to the treatment plan and documents all services provided.
-Conducts continuous quality improvement activities to assure caliber of service.
-Serves as agency liaison to other service and community organizations as assigned.
-Participates in training and consultation on HIV mental health issues to healthcare center staff, clients and the general public as assigned.
-Prepares monthly reports for the Office of AIDS Programs and Policies (OAPP) as required.
-Prepares quarterly reports for Positive Healthcare.

AHF Meetings:
Mental Health Team conference meetings

Other Qualifications:
-Minimum of one year of inpatient healthcare experience desired.
-Two years of clinical work, preferably with emphasis on chronically ill patients.
-Must be able to travel 20% of the time.
-Strong organizational and record keeping skills.
-Refined interview techniques.
-Knowledge of HIV/AIDS related issues and concerns as they relate to the diverse populations.
-Familiarity with organic and behavior disorders, dementia,
crisis intervention techniques and social/family systems.

Other Skills and Abilities:
-Bilingual (English/Spanish) skills strongly preferred.
-Strong interpersonal and communication skills.
-Ability to work as a team player.

Education and/or Experience:
-Master's degree (MSW) in Social Work or equivalent from an accredited school of Social Work.

Computer Skills:
To perform this job successfully, an individual should have knowledge of MS Office Word Processing software.

Certificates, Licenses, Registrations:
-Valid California Driver's License.
-Proof of liability insurance.
