San West Collaborative Position

Program Manager, San West Collaborative
MSW or MA (license-eligible)

The San West Collaborative (SWC) is a multi-agency project that provides permanent supportive housing to homeless people living with severe mental illness or co-occurring disorders in two apartment buildings. The goal of the SWC is to develop a financially sustainable and replicable service model that integrates housing with on-site primary care and behavioral health services as a vehicle for systems change to improve the well-being of people with special needs. SWC is a project of A Community Friends (ACOF), Lamp Community, Special Services for Groups and T.H.E. Clinic, Inc.

Position Summary: This position is an exciting opportunity to lead a diverse multidisciplinary team and manage day-to-day clinical and program operations of a model project that offers persons living with a severe mental illness and co-occurring disorders immediate housing combined with integrated health and social services including primary health care, psychiatric services, mental health counseling, drug/alcohol recovery, medication management, wellness and recreational activities, budgeting assistance and job training/placement.

Responsibilities:
Supervise staff using a collaborative team approach. Maintain oversight to ensure that each tenant’s self-identified goals for recovery; rehabilitation and community integration are supported by appropriate treatment, services and best practices. Help train interdisciplinary staff and in assist with staff hiring, development and evaluation. Monitor and evaluate performance and effectiveness of housing and services. Ensure timely data collection and information management including assessment forms, progress notes, treatment plans, and other data collection systems for outcome reporting and billing (HMIS, Caminar, Medi-Cal). Ensure compliance with program and contract benchmarks and outcomes. Prepare reports internally and for funders. Provide community education. Develop partnerships to leverage additional resources for tenants. Assist with program development and participate in project’s intra-agency management team. Assure fidelity to SWC model which emphasizes housing first, wraparound treatment and support services and customer choice to help men and women living with severe disabilities succeed in housing and progress toward recovery and wellness. Qualifications: MSW or MA (license eligible) required. Min. 3 years program management and supervisory experience. Demonstrated success managing programs for low-income persons with severe mental illness and addiction disorders in community based setting or in permanent supportive housing. Strong leadership, team building, clinical and program management skills. Demonstrated knowledge and experience of DMH/Medi-Cal billing. Bi-Lingual English/Spanish preferred. Advanced computer skills, including Microsoft Word, Outlook, Excel and database applications. Clear and effective written and verbal communications.
