Patient Navigator

The American Cancer Society (ACS) has an opening for a Patient Navigator (PN) located at Harbor-UCLA Medical Center in Torrance, CA. The PN meets face to face with cancer patients/families to help navigate the Harbor-UCLA system and provide information about cancer the disease, ACS programs/services, and community resources. Responsibilities include using tracking systems to monitor/evaluate PN Program. The PN acts as a representative of ACS and works cooperatively with the Oncology Team and the Social Work Department. Additional responsibilities include assisting with recruiting, training and supervising volunteers who provide support to the program.

Bilingual in Spanish with 3-5 years experience in a health care environment. Master’s degree in social work, public health, nursing, public administration or related field is preferred. Strong problem solving, relationship building, organizational, empathy, and advocacy skills, as well as knowledge of health care systems, community resources, medical terminology, and HIPAA. Experience working with oncology patients and families is preferred.
