Specific Education, Certification and Skill Requirements:
Bachelors degree in Social Work or related field is required.
Masters degree is strongly preferred.
Minimum of two (2) years of case management or related experience is required.

Job Description:
Skid Row Housing Trust is a nationally-recognized nonprofit developer of aesthetically designed, permanent supportive housing for very low-income and formerly homeless single adults in the Skid Row area of downtown Los Angeles.

The Trust is seeking highly skilled and motivated Resident Service Coordinators (RSC) to provide comprehensive case management in a residential Shelter Plus Care setting to homeless persons with one or more disabling condition (severe mental illness, substance abuse, physical disability, and chronic illness including HIV/AIDS).

Responsibilities:
„X Provide crisis intervention, intensive case management, and follow-up services
„X Perform comprehensive needs assessments and develop case plans with residents
„X Conduct group case management activities
„X Initiate, monitor, and follow-up on referrals to community partners
„X Provide assistance with accessing benefits and entitlements
„X Maintain accurate, up-to-date client records, statistical information, and reports
„X Participate in all staff meetings, on-going training, program evaluation, and program development and implementation activities

„X Ability to work independently and as part of a team
„X Solid computer skills
„X Excellent organization, interpersonal, and communication skills
„X Working knowledge of government and community assistance programs


Other Information About This Job:
„XDemonstrated knowledge of case management and issues affecting homeless residents with disabilities (severe mental illness, substance abuse, physical disability, and chronic illness including HIV/AIDS).
„XKnowledge of and ability to work effectively in a harm reduction environment.
